Areas of Specialty Include • Initiation, monitoring and weaning off tube feeds • NG-tubes/NJ-tubes • G-tubes/GJ- tubes • Formula selection • Blended tube feeding Initial consultation $150.00 per 60-minute initial consult Each initial consult includes: Review of 3-day diet history and health history intake Realistic and creative advice Customized nutrition care plan for your child’s specific nutritional needs Summary of visit, and next steps (nutrition care plan) Additional helpful resources Summary note sent to your child’s family doctor or pediatrician, upon request (additional fee) Follow up visits $75.00 per 30-minute follow-up $95.00 per 45-minute follow-up $150.00 per 60-min follow-up Each follow up session includes: Review of your child’s diet and progress Evaluation of the nutrition care plan Additional advice and suggestions to meet your child’s specific nutritional needs *** a minimum of 2 sessions –required 4 (1hr) session package $500 All sessions can be booked as phone or virtual consults We encourage you to check with your insurance provider about coverage for a Registered Dietitian or Allied Health Professional services
